APPOINTMENTS

SCHEDULING AN APPOINTMENT:

New patients: Please call the office where you wish to establish your care.  We request that you arrive 15 minutes ahead of your scheduled appointment time to allow time to complete the necessary registration forms.

All patients are highly encouraged to choose a primary office and a primary care provider at that office.  This will help us provide you with better, more consistent care.

Please call your primary clinic to schedule your appointments with your primary care provider.  Your primary clinic is the office that you use most frequently and is also the office where your medical chart is located.

Appointments will be scheduled with your primary provider whenever possible.  If your primary provider is not available or is unable to see you, you will be scheduled with another provider at your primary office.

You must bring your insurance card to all appointments to ensure proper billing.

Our computerized scheduling system helps us make a convenient appointment for you in a timely manner.  If you should call during a peak time, you may be asked to hold for a short time.

LAB / NURSE APPOINTMENTS:

An appointment and a provider's order are required for all laboratory tests and nurse procedures.  This appointment should be scheduled at your primary clinic whenever possible, since that is where your medical chart is located.

Please call at least 24 hours in advance to schedule these appointments.  This will enable us to have your chart pulled and ready when you arrive and your wait time should be minimal.

Lab appointments should be made for:

• All blood tests
• Strep tests / cultures
• Urinalysis tests

Nurse procedure appointments should be made for:

• All injections including Allergy shots, Depovera shots, Flu and Pneumonia shots
• Dressing changes ordered by a provider
• Ear lavage ordered by a provider
• BP monitoring

CANCELLATIONS:

Your appointment time has been reserved for you.  If you are unable to keep your appointment, please notify our office at least 24 hours in advance so we can offer that appointment to another patient who is seeking medical care.
